It's definitely easier to maintain input/new if there is no
subdirectory, so it's worth a bit extra coding in makelsr.py.

Tag 'other' in input/new has been junked.

Snippets in input/new have been cleaned up and/or updated in LSR,
further checking and cleaning up may be needed.

Remove LSR snippets (concurrent updates)

We no longer have .cvsignore in the sources.

Deeply revise input/lsr structure, generation and compilation

input/lsr has no more subdirectories, and snippets are glued into a
single Texinfo document.

More precisely, what is in this huge commit:
- buildscripts/makelsr.py is rewritten,
- input/new/*.ly are not compiled any more,
- tags are changed to match existing LSR tags (excepted 'other'),
- lys-to-tely.py is changed to fit more usages, e.g. generating
input/lsr/*.itely,
- makefiles, buildscripts, documentation index, snippets linked from
docs are changed accordingly.

MusicXML: Don't crash on multiple triplet children

Finale seems to produce xml files that contain <notation> tags
with possibly multiple <tuplet...> tags, in particular when
a tuplet is only for one note (with tremolo subdivisions)...

This patch only fixes the crash, but does not handle the logic behind
this correctly :-(
