Doc: obsolete need for skeleton files. Fixes `make website' warnings.
Use map source->translationof node names to rewrite any untranslated usages
of already translated nodes. This occurs eg after a [2] delivery of the
Dutch site, where the untranslated
news.itexi has @ref{Changes} and
community.itexi has @ref{Manuals} and @ref{Old downloads}
while these are translated in
nl/web/manuals.itexi Veranderingen @translationof Changes
nl/web/manuals.itexi Handleidingen @translationof Manuals
nl/web/download.itexi Oude downloads @translationof Old downloads

Context mods stored in variable, can be inserted into \with or \context
-) Context-Modifications: create C++ class to store them
-) context modifications lists are stored in a dedicated simple scheme object
(C++ class Context_mod)

-) \with blocks no longer return a simple list, but a Context_mod object
-) context_modifications objects (stored in variables) are now also allowed
with \with clauses

-) this allows us to rewrite \RemoveEmptyStaffContext (unfortunately with
a little different syntax, since we no longer store \Staff inside the
\RESC command) so that it no longer erases previous settings to the
Staff context. Now, instead of

one can do
\context { \Staff \RemoveEmptyStaves }
with the same effect and preserve previous changes to the Staff context.
(The same applies of course to \DrumStaff, \RhythmicStaff, etc. as well)

Add lilypond-book ly verbatim gettext quirk
Documentation/snippets/jazz-combo-template.ly contains a variable
definition named "bass", of which lilypond-book translates all
occurences in Texinfo verbatim ly code, including the case where
"bass" is an argument of the \clef command. As variables may be used
in Scheme code and this gettextization is done without real parsing,
it's impossible to avoid such corner cases, which will thus be fixed
ad-hoc as they are discovered.

Use iterator for STL std::map element access.
The at() function for std::map was added for libstdc++ release 4.1, and
it is not part of the official STL.
Also some developers are still using libstdc++ 4.0, so using at() will
not compile.
Instead, use an iterator for std::map element access, which is the
standard (albeit annoying) practice.

Pango: API change to fix fetaText rendering.
The changes to the Font_metric API rendered obsolete the "tight-bbox"
filtering in Pango_font.
This commit modifies the API appropriately to fix the rendering of text
with the fetaText encoding.
