The original images had a height of 36px; these new images have a height
of 100px and simply fill the remaining space with the "terminating"
gradient colors.

I don't know if this is really what we want for the lily-home images,
but this is acceptable for now.

By modifying the stems accordingly, they do not influence the

Allow unsetting Voice.instrumentCueName

Don't assume that Voice.instrumentCueName is a valid markup! If it is
empty (i.e. it has been manually unset), don't create an InstrumentSwitch

New `vspace' markup command, after Nicolas Sceaux

This commits adds a new builtin-markup-command,
as used by Nicolas in his scores.  This command
takes a positive number as argument, and makes
an invisible stencil of three times this number
(which is somehow equivalent to the vertical space
taken by a character).

The command definition includes a docstring and
an example, and changes.tely has been updated
accordingly.

One or both of these macros was breaking the build for the German NR
PDF.  There are other places in the docs with @rlearning commented out
for similar reasons, I think.

It seems that the custom @lilyvindex macro sometimes fails when its

lilypond-book: Change md5 hashing strategy

Previously, the md5 hash for folder/file names was based on the entire
.ly file contents.  This worked okay until the preamble changed, which
then broke `make check'.

This commit changes the strategy so that only the LY code (without the
preamble) is used when computing the md5 hash.  This will fix the regtest
comparison output between releases from now on.
