The code did not handle face indices; it relied on the PS name and the
(physical) font name only.  No visible changes for users.  However,
`ly:pango-font-physical-fonts' now returns a three-element list instead of a

`framework-ps.scm'.

In Paper_book::output_aux(), for a parent Paper_book, despite the
looping:
  for (SCM p = scm_reverse (bookparts_); scm_is_pair (p); p = scm_cdr (p))
not all children bookparts are processed, causing the bus error to
occur later on.

As a workaround, bookparts will be stored in Paper_book in straight
order (in Book::process_bookparts), instead of reverse order, to avoid
the multiple reversings (in paper_book.cc).

Add a hack to packed spacing to work around extra columns.

The ligature engravers leave unused paper columns lying around
and it's up to the spacing engine to deal with them. Here we
add a test for these extraneous columns and ignore them in
the packed-spacing part of the spacing engine.
