Further lilypond-book cleanup:

* Remove psfonts support.

  Users should include fonts with

  gs-load-fonts (optional)
  eps-include-fonts

* Remove output name support: we only support output dir now.

* Share lily-XXXX files for web compile in topdir/out/lybook-db
