Don't test for positive \lilypondpaperlinewidth -- this is always
positive in the normal case (LilyPond produces bad output otherwise
so we can safely ignore negative or zero values).
(\lilypondend): Remove useless test for \lilypondbook.
(\fetasixteen, \fetafont, \fetachar): Removed, unused.  We also
don't input `feta20.tex'.
