texi2html: Clean up the file, add comments/documentation, rearrange functions

Add documentation/explanations to the file so other devs can also understand
what it's doing.

texi2html: Logic to load translations (secname -> filename map)

In the future, section names will be translated directly in the
.tely file, with a @translationof command indicating the original
section name (which will be used for the file name). Since we are
also putting several sections into the same file, the file name
can no longer be obtained from the section name only. Instead,
we need to keep a map of secname->filename#anchor.

This commit adds the general structure of the logic to look up the
correct file name and anchor for sections, although loading
and reassigning the new values is not yet implemented.

Get 'line-positions and line_count from staff symbol grob in Staff_symbol::on_line ().

This prevents dots and spanners from being positioned incorrectly.

texi2html: Create tab-separated file containing the NODE<=>FILENAME map

For each main html file, I now write out the map of filename<=>nodename
associations to the file basename_xref.map (where basename is e.g. index).
This file contains tab-separated lines of the form

NODENAME\tFILENAME\tANCHOR_IN_FILE

In the future, this association will be used by the other files to
properly resolve external references. Of course, this will require processing
each texi file twice...

Texi2html: Fix displayed text for external links

The default formatting for ext links was
  "See <a href=..>NODENAME</a> section 'SECTION' in BOOKNAME"
We only want <a href=..>NODENAME</a>, so I override the default
behavior with our much simpler one and don't call the default
handler any more
